To receive the health insurance wellness premium in 2019, you are required to earn 400 points in the Viverae/Simply Well portal by December 1, 2018. To verify your point total, login to your account at https://connect.viverae.com. Select the drop-down triangle and change your view to ‘2019 BVU Premium Incentive’. Your goal is to have 400 points earned. If you don’t have 400 points, click on ‘Earn More’. Review the information on the screen to learn how you can earn extra points. You may need to watch a short video or participate in a 4-week program, so you don’t want to wait until the last minute.

If your spouse/domestic partner participates in the wellness program, you will want to confer with him/her to make sure he/she has also earned 400 points.
