Eliminate a Confidential Credential File

I no longer need to maintain my confidential credential file at Buena Vista University and therefore give permission to the Office of Career Services to destroy all confidential information contained in my credential file. I understand that I waived my right to view or possess copies of the confidential letters when I set up the file.

If there are any non-confidential materials in this file, please mail those materials to me using the information below:

Please allow up to 7 business days for requests to be filled. We will not assume liability for files once they have been placed with the U.S. Postal Service.
